Course Information:
The course is about “Person Centred Planning”.
It will run over 5 sessions.
By the end of the course, you will have a toolkit
which will help you to understand
what a “Person Centred Plan” is and how to make
one!
Homework Task – For Session 2 you will be set a
homework task to think about 5
things which are important to you.
You will be asked to bring 1 in.
This can be a photograph or an item of something
that is important to you.
Please note that transport will not be provided
to and from the sessions.
